What a Proper Testosterone Check Looks Like
A single blood draw in the afternoon is not what the guidelines describe.
Picture a man named Greg, 61. He has felt tired for a year. A friend told him to "get his testosterone checked." He is not sure what a good check involves, so he asks a few questions before he goes.
What the guidelines ask for
Two major guidelines describe the same basic steps.
| Step | American Urological Association | Endocrine Society |
|---|---|---|
| Symptoms or signs first | Yes | Yes |
| Early-morning blood test | Two separate tests | Fasting morning test, repeated to confirm |
| Cutoff for low | 300 ng/dL total testosterone | Consistently low level |
| Routine testing for everyone | No | No, they advise against screening |
The Urological Association guideline was published in 2018 and confirmed as valid in 2024. The Endocrine Society guideline is from 2018.
What this means in practice
A single blood draw in the afternoon is not what the guidelines describe. Testosterone falls through the day, so a late test can read low in a man whose morning level is fine. A second reading guards against acting on one low number.
A good visit could also include a talk about sleep, alcohol, weight, medicines and mood, since those can cause the same symptoms.
Questions Greg could ask
-
Will you test in the early morning?
-
Will you repeat the test if it comes back low?
-
What else could explain how I feel, and will you check for it?
-
If the level is low, what are the benefits and risks of treatment for me?
You could…
-
Ask for the first blood draw before 10 a.m.
-
Bring your written list of symptoms and when they began.
-
Ask what the plan is if the result is normal.
