From Housekeeper to Nurse: Who Could Help, and What Each Does
Is a nurse the same as a caregiver?
Picture a woman named Dottie, 76. Her husband walks slowly and needs help to bathe. She has done most of it herself in their Mexico City apartment. One friend told her to hire a nurse. Another told her to ask the housekeeper for more hours.
"Is a nurse the same as a caregiver?" Dottie asked a neighbor.
"The price lists I saw had them on different lines," the neighbor said.
Dottie went looking for the lines. This is what she found, in pesos, with dollars at 17.96 pesos to the dollar (Xe, 6 October 2026). These are quotes from price pages and marketplaces in 2026, not statistics.
| Level of help | What it generally means (our reading) | Pesos | About US$ |
|---|---|---|---|
| Housekeeper | Cleaning, cooking, laundry | About 125 an hour (my family's figure) | About 7 an hour |
| Private day caregiver, 8 hours a day | Help with bathing, dressing, meals, company | 7,500 to 13,000 a month with social security and extras | 418 to 724 |
| Caregiver from an agency | The same, with a company behind the person | 200 to 350 an hour | 11 to 19.50 an hour |
| Live-in caregiver | One person who lives in the home | 18,000 to 32,000 a month, employer's total | 1,000 to 1,780 |
| Home nurse, marketplace | Tasks that need nursing training | 118 to 200 an hour; 1,232 to 1,600 for an 8-hour day | 69 to 89 for the day |
| Geriatric nurse, agency | Nursing care for older adults | 400 to 800 an hour | 22 to 45 an hour |
Here is one way a family might combine them. Dottie priced a private day caregiver at 7,500 to 13,000 pesos, plus a nurse for a basic two-hour visit twice a week. The marketplace listed those visits at 240 to 400 pesos each. Eight visits a month come to 1,920 to 3,200. The total is 9,420 to 16,200 pesos, which is about US$524 to US$902 a month.
In my own family, most days my father needed little more than our housekeeper gave us. Caregivers carried the rest.
A few notes came with the prices.
Mexican law has no separate category for caregivers and housekeepers. A law firm summary says caregivers of older adults fall under the same rules as other domestic workers.
A national skills standard exists, called EC0669, for basic care of older adults at home. One organization announced in December 2025 that it had certified 20 domestic workers. How many caregivers hold it, and whether agencies ask for it, is a question for the agency.
What license Mexico asks of a home nurse is not in these price pages. Ask the office, or a paid professional. A family could ask to see it.
On what each level may do, the following is our inference. A 2017 chart from the Illinois health department puts help with daily tasks, housekeeping and medication reminders on the non-medical side. It puts giving medication, dressings and tube feeding on the nursing side. Mexico may draw the line elsewhere, and a doctor can say which tasks need a nurse.
You could…
-
Ask the doctor which tasks in your family's day need nursing training.
-
Ask each agency what training its caregivers hold, and whether any have the EC0669 certificate.
-
Write down a typical day, hour by hour, and mark the level of help each hour needs.
